THANK YOU FOR YOUR SUPPORT OF THE TELETHON OF STARS.

We are now accepting "Talent Forms" for the 54th Annual Telethon of Stars set for Nov. 13th & 14th
at the Carson Center in Paducah, KY.

Click HERE to download the "Talent Form" (pdf file), fill it out completely
and return with indicated documents to the address on the form.
Deadline for receiving your "Talent Form" will be September 1st, 2010.

If you submit a form and are accepted for the show, you will receive
a notification no later than October 1st, 2010.
